How to Make a Referral

APSI accepts referrals by a phone call or written request to the APSI regional program director at your local APSI office.

Presenting Situations for a Referral to APSI

APSI may accept an eligible person for guardianship or other services when:

  1. There is substantial risk because the individual lacks the ability to understand and make informed decisions and lacks adequate supports; and
  2. No other alternative meets the person's needs, or additional circumstances complicate the situation:
    1. A sudden change in physical condition requires medical or surgical intervention;
    2. An unexpected serious occurrence, such as an accident, has caused injury;
    3. A medical condition exists that left untreated could end in serious harm;
    4. A chronic medical condition requires ongoing decision for treatment;
    5. Challenging behaviors occur and are likely to require major aversive interventions, restriction of rights, and/or psychotropic medications;
    6. The individual is at risk because of alleged abuse, neglect, or chronic financial exploitation; or
    7. Multiple problems exist, no one of which alone justifies intervention but which taken together become significant.

Waiting List
APSI implemented a waiting list for services in June 2009 due to funding cuts. Each APSI region maintains a waiting list and individuals are served on a first come first serve basis, subject to certain exceptions.
